Chapter 6 Deviations

6.1 Introduction

Time and concentration deviations may require correction prior to completing final NCA calculations. This section describes methods used for corrections and the associated calculations to be performed prior to final calculations.

6.2 Limit of Quantification Modifications

At least one of the following rules must be implemented for handling concentrations below the limit of quantification (BLOQ). Different rules may be implemented for

  • the predose concentration measurement,
  • concentrations before the first concentration above the LOQ,
  • BLOQ concentrations between the concentrations above the LOQ, and
  • after the last concentration above the LOQ.

6.3 Time Deviation Correction Rules

The time deviation correction rules have to be applied in cases where the actual sampling time deviates from the nominal time at t=0 (all AUCs), t=t_upper (AUCINT) and/or t=TAU (AUCTAU).

6.4 Concentration Deviation Correction Rules

Concentration substitution rules have to be applied where the concentration is missing at t=0 (all AUCs), t=t_upper (AUCINT) and/or t=TAU (AUCTAU).

6.5 Interpolation

To obtain a concentration (\(C_i\)) at time i (\(t_i\)), lying between the times \(t_a\) and \(t_b\) with concentrations \(C_a\) and \(C_b\) respectively, and no dose occurring between \(t_a\) and \(t_b\) interpolation will be carried out. Multiple methods of interpolation are possible, and the method of interpolation should align with the method for calculation of area under the concentration-time curve (AUC).

6.5.1 Zero Interpolation

If \(C_a\) and \(C_b\) are BLOQ, \(C_i\) should be set to BLOQ.

6.5.2 Linear Interpolation

If linear AUC integration is used in the interval between \(t_a\) and \(t_b\) (trapezoidal rule AUC calculation or linear-up/log-down AUC with \(C_b \geq C_a\)), the following formula should be used.

\[C_i = C_a + \left(C_b-C_a\right)\frac{t_i-t_a}{t_b-t_a}\]

6.5.3 Log-Linear Interpolation

If log-linear AUC integration is used in the interval between \(t_a\) and \(t_b\) (linear-up/log-down AUC with \(C_b < C_a\)), the following formula should be used. \[C_i = exp\left( log\left(C_a\right) + \left(log\left(C_b\right) - log\left(C_a\right)\right) \frac{t_i - t_a}{t_b - t_a} \right)\]

6.6 Extrapolation

To extrapolate a concentration \(C_i\) at time \(t_i\) lying after a time \(t_a\) with measurable concentration \(C_a\) and no dose occurring between \(t_a\) and \(t_i\), the following formula should be used. \[C_i = C_a exp\left(-\lambda_z \times \left(t_i - t_a\right)\right)\]

where \(\lambda_z\) is the elimination rate constant and it must be positive. It is calculated during calculation of the half-life (\(t_{1/2}\)), and is calculable from half-life as \(\lambda_z=\frac{log_e(2)}{t_{1/2}}\).
